Restrictions on Material and Information: All materials, including without limitation, text, illustrations, trademarks, and all copyrightable material (including source code and object code), images, video, advertising, graphics, HTML, codes, multimedia clips, logos, button icons, banners and software, and any other form of intellectual property (“Materials”) contained in this site and database are the property of LifetimeAnalysis. LifetimeAnalysis reserves the right at any time and at its sole discretion to change or revoke 
any authorization to its subscribers. Your subscription to LifetimeAnalysis may not be shared, 
transferred, or redistributed to other healthcare providers, offices, or any other party. Your subscription is restricted to one user login with your username and password. If multiple attempts are made to login more than one user with the same username and password all users will be removed from the database. LifetimeAnalysis reserves the right to suspend your subscription if our tracking suggests attempts to login multiple users with the same username and password.

Nothing contained in this Site will be construed as granting any license or right to use any trademark, logo, trade name, or design displayed on this Site or otherwise owned by LifetimeAnalysis without the prior written permission of LifetimeAnalysis.

Limitation of Liability: The use of this Site and information contained within the Site is at your own risk. All subscription options will be automatically renewed unless we are instructed to perform a cancellation by email or in writing. A 30 day advanced notice is required in writing by email for cancellation. See the “Contact Us” tab on the website for contact information. If LifetimeAnalysis does not receive an email instructing us to cancel your account, renewal of the subscription will automatically occur for another subscription term at the current option level. It is the subscriber’s responsibility to keep us updated with any changes to their credit card information.

Miscellaneous: This Agreement shall be governed by and construed in accordance with the laws of the State of Iowa. You agree that any legal action or proceeding between LifetimeAnalysis and you for any purpose concerning this Agreement or the parties’ obligations hereunder shall be brought exclusively in a court of competent jurisdiction sitting in Polk County, Iowa. Any cause of action or claim you may have with respect to the Service must be commenced within one (1) year after the claim or cause of action arises. LifetimeAnalysis’s failure to insist upon or enforce strict performance of any provision of this Agreement shall not be construed as a waiver of any provision or right. Neither the 
course of conduct between the parties nor trade practice shall act to modify any provision of this Agreement. LifetimeAnalysis may assign its rights and duties under this Agreement to any party at any time without notice to you. Trademarks and service marks appearing on the Service may be owned by other parties including third parties not affiliated with LifetimeAnalysis.
